Financial commitment may be an important part of your decision to have cosmetic surgery. The following information is provided to you in advance as you investigate options for achieving your cosmetic surgery goals.

• There is a $50.00 charge for your initial cosmetic consultation.

• Following your consultation, you will meet with our Financial Coordinator who will review fee estimates and payment details for the procedures you discussed with Dr. Smith. You will receive a copy of the estimates at the time of your visit or within one week if not completed at the time of your consultation.

• A $500.00 deposit is required prior to any cosmetic procedure being scheduled at a surgical facility. The deposit is applied to Dr. Smith's surgical fee.

• For in-office cosmetic procedures, a $100.00 deposit is required prior to the procedure being scheduled. The deposit is applied to the surgical fee.

• Payment for Dr. Smith's fee for cosmetic procedures performed in a surgical facility is due at least two weeks prior to the scheduled surgery date. Payment for in-office procedures is due in full prior to your being prepared for the procedure in the office.

• Payment may be made by cash, cashier's check, personal check or credit card. VISA, MasterCard, and American Express are accepted.
